All spring sports physicals, registrations, and concussion screenings need to be done by March 9^th. No documents will be accepted after 3/9 and no documents should be handed into coaches. 
•    If the student did not play a fall or winter sport at Unami, submit section 5 and 6 of PIAA physical dated after June 1st, 2019 to Mr. Beyerle (can be dropped in the front office or scanned and emailed to gbeyerle@cbsd.org as well as uploaded via Family ID). If the student did play a fall or winter sport, complete section 7 of the PIAA Physical form and return.
•    Registered for appropriate sport via Family ID accessible through Unami Athletic Website (available currently)
•    Completed concussion screening in the past at Unami (track does not need a concussion screening)
